As head of operations and later chief technology officer at Etsy, he wrote the post that carried the just culture idea from safety science into software incident reviews: a postmortem that asks what happened and why, without blaming the person, so engineers report mistakes fully. He later earned a master's degree in human factors and systems safety at Lund University and co-founded Adaptive Capacity Labs.
